Extra Care and Support for People With Low Incomes or Certain Medical Conditions
Special Needs Plans (SNPs) are Medicare Advantage plans for people living with certain conditions or diseases, or those with low incomes. It can be hard to have a condition that requires extra health care and just as difficult to cover health costs on a fixed or low income. We’re here to support you with extra benefits, drug lists and care networks that fit your special needs.
Take Advantage of Extra SNP Coverage
If you have both Medicare and Medicaid, our Dual-Eligible Special Needs Plans (D-SNPs) include dental, hearing, vision and prescription drug coverage. You don’t need to pay anything for a routine teeth cleaning or eye exam. Many of our plans also cover hearing aids.
What’s Covered Under Empire’s Medicare Special Needs Plans?
Our Medicare Advantage SNPs include Medicare Part A and Part B, but they also cover what Original Medicare does not. There are benefits for routine care, and resources such as money for health-related items.
Valuable Benefits and Coverage in Our SNPs
- $0 copays for primary care doctor visits
- $0 copays for dialysis, lab services, diabetes supplies and more
- SilverSneakers® fitness membership
- Money for over-the-counter health-related items
- Personal emergency response system
This list does not include all SNP benefits, and remember, coverage varies depending on where you live. So, take a minute to find and compare Empire plans in your area.
Dual-Eligible Special Needs Plan and Eligibility
You’re eligible for a D-SNP if you have both federal Medicare and state-run Medicaid.
To be eligible for Medicare, you need to be 65 or older or have certain health conditions. Medicaid, on the other hand, is based on income levels instead of age. If your income qualifies you for assistance, you can be younger than 65.
We offer D-SNPs with dental, vision, hearing and prescription drug coverage, plus more. Learn more about our D-SNPs and the benefits that come with them.
Chronic Condition Special Needs Plan and Eligibility
Empire currently does not offer C-SNP plans but we may soon in the future.
If you don’t qualify for a SNP, our Medicare Advantage plans still offer similar benefits. You just need to be enrolled in Original Medicare to apply. Try our shopping tool to help you find and compare Empire Medicare Advantage plans in your area.
